It would depend on how it is organised. A typical fast food restaurant will have an owner/manager who will also have duty managers for each shift, and then team leaders for each segment and then those that do much of the work. All in a total number of people of 30 to 50 depending on size of store. Though you could probably ignore the team leaders as they are more likely to be simply senior workers on a slightly higher rate of pay. So could have simply three levels layers of staff, or two levels of management. Even with 500 staff you could simplify it considerably to a couple of levels. You would start at the top with the board of directors, then below that departmental managers. All at the same level basically but with different importance depending on the nature of the business. Then those managers will be responsible for up to a few dozen people, but with the odd team leader but depending on roles they may not be management.

Well in this case it goes...

HQ (CEO & directors)
Regional Manager(s)
Store Manager
Senior Management team (typically duty managers, head of HR etc)
Department managers.

And then supervisors, which we aren't including.

Obviously HQ apply to the whole company and not just one store, similarly with regional managers - but that's still 3-5 layers.

You may have missed out buyers and property and HR management above regional managers but these departments are usually very small with very short management chains.

Regional managers may be simply senior managers who act like team leaders for the store managers.

Though unless you are talking about a big chain store with big departments like a supermarket chain where the store manager has bakery; clothes; fresh produce managers etc then the chain may be shorter than you think. With people really being team leaders with a fancy title. A company like Carphonewarehouse my have

HQ (CEO & directors)
Senior managers/head of locastions/buying/HR/finance and other departments
Area managers
Store Manager

Many of the levels may not be true levels because they have no power and are simply supervisors.

Greavey train? I don't like that phrase too emotionally charged, gives false perceptions of that which should be looked at with scrutiny and logic.

Too much of a daily mail phrase...

And why would a hospital need a marketing director, it is not as if they need to drum up trade?

There's more to marketing than just getting people through the door, don't forget.

Yes but how often do they really need it? Maybe with the "internal NHS market" all we have done is create lots of jobs for marketing people trying to get our ops done in XYZ hospital.

Yep. Burying inconvenient truths, covering up mistakes, persuading people the dirty floors are imagined, (that's just the pattern on the floor suface, sir)...

Or is it that getting extra funding from the government requires a smarmy person to "sell" the hospital to the inspectors?

If I ever catch you apologising for being from the North again you'll get a clip 'round the bloody ear. :)

Just read in the metro - http://www.metro.co.uk/home/829784-spin-king-s-salary-tops-david-cameron-s-wages

Suffolk County Council bosses are set to pay a new spin doctor £3,500 a week – dwarfing the prime minister’s salary

I will do it for “only” :D £1,750 / week – there an instant saving to the council
