Someone is getting in trouble. There are strict rules on where you can dump fuel.
Presumably the pilots got permission from air traffic control. If so the controller is going to be investigated very closely.

Under optimal circumstances, on the other hand, if you have lost an engine, are losing height, can't climb and are too heavy to land, what are you supposed to do?

I don't know the exact circumstances here, but there are situations, where high altitude and designated areas don't even enter the equation...

All modern airliners should be able to climb on one engine, through the climb rate and maximum altitude will be lower than normal.
