When I first heard about this, I thought that the trainee must have missed some glaringly obvious warning signs. Then I learned a bit more and heard there were "system errors" and that this doctor was being made a scapegoat. Today, I found out some stuff:

  • The child was admitted with gastroenteritis
  • The registrar ordered fluids and arranged for some investigations
  • The child had improved and was moved to another ward
  • Somebody had given the patient a blood pressure medication. It hadn't been prescribed by the doctor.
  • The registrar was on her first day back from maternity leave in a new trust
  • There had been no induction - no introduction to the department, the layout, the computer systems. Nothing.
  • There should have been a second registrar and a consultant on the ward. They were both away.
  • There were two more junior doctors who had just started working on that ward.
  • The whole hospital's IT system was down. Results couldn't be seen on computer and one of the junior doctors was allocated to track down blood results by telephoning regularly.
  • When the IT system did recover, the warning system for abnormal results was down and so nobody had a clue whether there was an abnormal result.
  • There were nursing shortages at the time. A nurse with no training in nursing care was looking after the child.
  • The registrar was doing the job of at least four doctors on her first day in a new department with no coverage.
  • The only issue was the registrar had mistaken the child for another one and stopped CPR. It hadn't contributed to the final outcome but no one had told the registrar that patients had been moved around. None of the more senior doctors present challenged the decision to stop CPR.

is it the management looking for a scape goat. as the medics seem to have supported this doctor ...

it would seem overloading the NHS with management is working to the detriment of the medical profession directly ...
